Q.1: What are the membership requirements?
All Muslim adult Canadian Citizens, Permanent Resident Card, Super Visa, Student
Visa and those legally staying in Canada and their family members are eligible to
register as a member of KWMF (Funeral Support Services Organization).
Q.2: What is the Membership fee structure?
KWMF FEE STRUCTURE
KWMF fee structure is based on once and lifetime membership fee (non-refundable) as follows:
Principal Member ……………………………………$100 + $25 Admin Fee = $125
Spouse………………………….……………..…………………………………………………….= $25
Each Dependent child under 19 years of age………………………….……= $25
Child over 19 years has to fill up a separate Membership Application.
Form and fee is same as Principal Member …………………………………=$100 + $25 = $125.00

Q.3: Is there any payment I have to make once I become a registered member of KWMF?
Yes, you have to make only following three payments:
- If any registered member of FSSO passes away then only you, as a Principal Member, will be required to proportionately share the funeral expenses incurred for him/her to reimburse the stabilized balance.
- As unanimously agreed by the Management and all registered members of FSSO, an amount of ($3.50/month X 12 month=$42.00/year) will be withdrawn once in each year on 1st January being contribution of members for operational expenses. The share may vary from year to year based on prevailing inflationary situation and cost of living at that time.
- In the event of any Mail, Email, Phone number and Bank Account transactions are returned due to changes made in them by the member without notifying us, FSSO will automatically withdraw $25.00 from their account as penalty.
Q.4: What documents are needed to register as a member?
You are required to submit the following documents:
1) Membership Application Form
2) Canadian status Photo ID (We do not accept Driver’s License, Photo Card and Health Card as a proof of Canadian status ID.)
3) A Cheque of Membership Fee or Proof of E-transfer
4) A void cheque or print out of your bank account
Q.5: If I move to any other city of Ontario Province will I still be entitled for the benefit of KWMF services?
Yes, you will continue receiving benefit of KWMF services as long as you are within Ontario Province area.
Q.6: Will I be eligible for full KWMF services immediately after my registration a member?
As per established policy of KWMF the membership activation period is three months from the date the application form is received. After three months the membership will automatically be activated for full KWMF services.
Q.7: During my membership activation period will you deduct my share of reimbursement in the event of demise of any KWMF member?
Yes, as a registered member, only Principal Member will be required to pay share of reimbursement.
Q.8: If any registered member passes away before the completion of three months activation period what services KWMF can provide?
Unfortunately, KWMF can not provide any funeral financial support services.
Q.9: Can principal applicant add the name of parents in his/her application form?

Q.10: In the event of KWMF member’s death, what procedure should be followed?
Death at Home or in the Hospital, First obtain the death certificate from Hospital or Health authorities. Now call FSSO for burial arrangements. Call 24/7 Emergency Number 289 946 1511 (Strictly…….only for death information). All steps of funeral processing of FSSO deceased Members are taken precisely following the guidelines of Muslim Faith (Sunni).
Q.11: If a member of KWMF becomes defaulter by not paying his share of reimbursement cost how the matter is handled by KWMF?
KWMF will revoke his/her membership.
Q.12: If a registered KWMF member passes away out of Canada will FSSO still provide funeral financial services?
No. Our services are available only in the Province of Ontario.
Q.13: I am a registered Principal Member of KWMF. One of my child who was also registered as a child member of KWMF at the time of my registration has reached the age of 19–what next I have to do?
You have to fill up a separate Membership Application Form for him/her which can be downloaded from our website www.kwmf.ca and send to us with $100 Membership Fee ($75 + $25 Admin Fee) to register as Principal Applicant. Please make sure that you or your child (if has a separate bank account) fill up and sign the “Pre-Authorization Payment Plan Agreement and attach a VOID cheque.
Q.14: If a registered member passes away in any city of Ontario other than city of his/her residence, will KWMF transport the body to his/her city of residence for funeral services?
No, KWMF will not be responsible for transportation of body.
Q.15: If the applicant and his/her family living in remote area out of GTA how KWMF will make funeral arrangement and payment there if any member passes away.
The family must look for a Masjid with funeral arrangement and Cemetery where Muslim Section is available and notify us. For the payment of funeral cost up to $7,000 minus applicable overhead expenses, if the Masjid and cemetery accept the payment by Credit Card, Email Transfer and Cheque, KWMF will make the applicable payment directly to them upon the receipt of Invoices from them in the name of “Funeral Support Services Organization, 215 – 50 Steeles Avenue East, Milton, ON, L9T 4W9”. In case of payment made by the Member, ensure that the invoice/s are issued in the name of “Funeral Support Services Organization, 215 – 50 Steeles Avenue East, Milton, ON, L9T 4W9” and submitted within 10 days from the date of burial, otherwise the funeral cost will not be reimbursed after 10 days. Funeral cost invoice/s of the deceased KWMF Member issued by the Cemetery and/or the Masjid in the name of a person is/are not acceptable at all and no reimbursement will be made against such invoice/s by the management of KWMF (Funeral Support Services Organization) .
Q. 16: If the new applicant Principal Member does not include the name/s of his/her spouse and children under 19 years at the first stage when submitting the Membership Form for the reasons known to him/her can he/she add their name/s later on.
Yes, they can include them by paying membership fee of $25 + $ 25 admin fee each and by filling up the “Amendment Form” available on KWMF website www.kwmf.ca, but they will also be subject to 3 months activation period from the date their name/s included in the dependent list of principal applicant.
Q 17: What is the best way to submit Membership documents?
There are two options you may submit the documents.

Go to our website “ www.kwmf.ca “ click on Membership Application and fill up the form, attach photo ID, proof of membership fee payment, copy of void cheque or print out of your bank account.
2. DOWNLOAD PDF. FORM
Go to our website, download Pdf. Form, fill up the form, sign at the required places, attached all needed documents mentioned in Question 3 and mail to us at given address: 215 – 50 Steeles Avenue East, Milton, Ontario, L9T 4W9.
Q 18: Can a registered Principal Member of KWMF cancel his/her membership?
Yes, only he/she can cancel his/her membership when they want to, without any obligation. They will be required to complete the “Amendment Form” which is available on the KWMF website www.kwmf.ca. Furthermore, if the Principal member terminates their membership then the memberships attached to him/her will also be terminated.
Q 19: If a Magnetic Membership Card lost by any member can it be replaced.
Yes, Magnetic Membership Card can be reissued with the cost of $30.00.
Q 20: Can the Membership be transferred to any other person?
No, the Membership is not transferable at all to any other person.
Q 21: After submitting Membership Application when we should expect Membership confirmation.
If all required documents are attached and correct, a formal Acknowledgement letter will be sent within 4 – 6 weeks.
Q 22: Do the Funeral Support Services Organization claim death benefit from the government.
No, we do not claim any death benefit from the government.
Q 23. If a member of KWMF declared defaulter and his/her membership cancelled, can the same be reinstated after the dues are settled?
No, for six months neither his/her membership will be reinstated, nor they can apply for new membership from the date they were declared defaulter. Also the memberships attached/sponsored by him/her will also be terminated.
